Conrad has a massive falling out with Jeremiah
To summarise: Conrad is not impressed that Jeremiah cheated on his fiancée. Jeremiah is not impressed that Conrad confesses eternal love to that fiancée. They also argue over a letter from their mother. The brothers actually fight. Then Jeremiah dramatically declares: “You’re not my brother anymore. You’re dead to me. I don’t want you at my wedding. I don’t want you in my life. I want you gone.”

Conrad bangs on about becoming a doctor all throughout The Summer I Turned Pretty books and show. In the book We’ll Always Have Summer, while Conrad is studying medicine at Stanford University, he works part-time in a medical lab. But we don’t actually find out whether Conrad achieves his dream. The epilogue of the final book spends lots of time describing Belly’s outfit, and Conrad’s skin through this shirt, but makes no reference to that huge unresolved storyline. Of course.

Yup, Conrad does end up with Belly
While Belly is studying in Spain for her junior year, Conrad starts posting her actual hand-written letters each month. Belly initially ignores them. Rude. He then shows up a year later at her college graduation. The book does not actually clarify whether Belly actually invited her ex-fiancé’s brother to her college graduation, or whether Conrad just decided to rock up and cause drama (I reckon he did that, given his track record).

Belly then decides she does like Conrad after all. They get married when Belly is 23, and Conrad is approximately 26. It rains at their wedding and they dance to Stay by Maurice Williams and the Zodiacs.
The whole saga ends with the words: “This is our start. This is the moment it becomes real. We are married. We are infinite. Me and Conrad. The first boy I ever slow danced with, ever cried over. Ever loved.”

It’s implied Conrad eventually patches things up with Jere
Well, Jeremiah was invited to Belly and Conrad’s wedding. So I suppose Conrad and Jere can’t be on completely terrible terms anymore?
